It helps if the pants have pleats.
I put the front suspender buttons so that one of them is directly over the main pleat (the one that is connected to the crease that runs down the center of the front of the pants). If you have no pleats, then try to center the button over the crease. The suspender tension will make the crease hang beautifully.
The second front button goes to the OUTSIDE of this first button -- it is placed further away from the zipper. Suspenders should not run down the center of your pectorals, but should be slightly off to the sides.
The distance between the two buttons is a matter of taste. I usually shoot for about 3 inches. If you have a second pleat, you might try putting the second button directly above it, just like the first. That way, both pleats will have maximum vertical tension.
In the back, the two buttons are also about 3 inches apart. A little further apart is fine too, if you want, and might add a little stability. As long as they are not so far apart that the waistband bunches up, it's fine. The suspenders in the back are covered by a jacket, so this is more functional than aesthetic.
